Patients Requesting Medication

Patients can make repeat prescription requests using 3 methods:

  1. Local pharmacy request
  2. Website prescription request form
  3. Patient Access application

All medication requests will be actioned by the GP within 2 working days

  1. Local Pharmacy Request:

    Any patient on repeat medication with a nominated pharmacy can contact their pharmacy to request their medication on the patient’s behalf. 

2. Website Prescription Request Form:

Any registered patient can use our prescription request form on the practice’s site: https://graysinnmedical.co.uk/prescription-request-form/http://vauxhallsurgery.co.uk/prescription-request-form/.

This form asks patients for their name, date of birth, address, contact details, medication required and nominated pharmacy. Patients can also add a message for the GP if it is required.

3. Patient Access application

Patients who have access to Patient Access can request repeat medication using the application.
